- Advanced Practice Provider/NP/PA
Description
The APP/NP/PA role is someone that possesses advanced knowledge and skills to
complement the care provided by our Medical Director. Candidates must be
experienced APP/NP/PA that are passionate about patient advocacy, care coordination,
and osteoporosis management. The APP/NP/PA will be responsible for patient
evaluations, testing, diagnosis, plans of care, and treatment. They will work with our
administrative staff and systems to coordinate patient care transitions from inpatient
hospitalization to the outpatient setting, specifically targeting musculoskeletal injury and
fracture care patients. The APP/NP/PA will function in an advanced practice role as a
consultant, facilitator, educator, change agent, program designer, outcome manager,
and resource person to the orthopaedic trauma patient population and caregivers. This
position assumes a leadership role in strengthening collaborative relationships with
healthcare leaders, physicians, advanced practice providers, and patients in various
settings and shifts, to improve patient/family outcomes along the care continuum.
Requirements
Minimum Position Qualifications:
Education:
-Graduate of an approved Advanced Practice training program
-Current with respect to continuing education requirements
Experience:
-Minimum of 3 years of experience in a clinical setting, preferably in geriatrics,
orthopedics, or chronic disease management
-Background in case management, care coordination, or patient advocacy
-Familiarity with musculoskeletal injury treatment and osteoporosis management
Preferred
-Non-orthopaedic experience will be considered on a case by case basis
Demonstrable Skills:
-Excellent interpersonal, communication, and organizational skills
-Strong entrepreneurial mindset, comfortable in startup environments
-Ability to manage and grow relationships with hospital case management
departments
License / Certification:
-Current California NP or PA license
-BLS certification, DEA License
Payer Credentialing:
-Ability to successfully complete the credentialing process by third-party payors and
Ability to enrolled as a CMS Medicare/Medicaid payor
