Description

 Collaborate with the Clinical Team for Person Centered Services.
Review and provide feedback regarding Life Plans upon request.
Provide consultation on psychiatrically complex cases.
Provide clinical guidance regarding support or treatment options for substance use or chronic health conditions.
Review of and consultation regarding hospital discharge summaries when requested.
Consult with care management teams regarding transitional care.
Facilitate a brief clinical discussion between the care manager and member regarding transitional care upon request.
Consult with nursing support specialists regarding medication regimens and medication options upon request.
Responsible to provide support and consultation to the after hours on call staff, as needed
Other duties as assigned/requested.

Requirements

 Possession of a license to practice medicine in the state of New York.
Board Certified in Adult Psychiatry and/or Child and Adolescent Psychiatry.
Knowledge about OPWDD services and regulations.
Strong computer knowledge.
Familiarity with person-first, person-centered planning concepts.
